BZ Management has one application form and two separate lease forms for Champaign rentals and Urbana rentals. The following lease forms are representative.
Lease Forms
The Champaign and Urbana lease forms are just examples for your information. Once your application is approved, you will be given a formal lease to sign.
Application Form
Please fill out both pages of the form and print them. To fill out the form in your computer, you will need Adobe Reader. (Get Adobe Reader). Right-click each link and choose "Save Linked File to Downloads". Go to Downloads in your computer, and open the pages of the form with Adobe Reader.

Send both pages of the form to the address at the top of the form with your $20.00 application fee and copies of ID's.
